Continuous Monitoring
Certification is a decision. Monitoring keeps the evidence moving.
An AI system can change after certification. Models evolve, deployments expand, data shifts, controls change, and new incidents can alter the risk picture. Clause5afe continuous monitoring is designed to extend qualified evidence collection across the certification validity period without replacing independent human judgment.
After the decision
A certificate should not freeze the system in time.
Initial certification evaluates a defined system, version, deployment context, intended use, and evidence set. Continuous monitoring helps identify whether conditions relevant to that decision may have materially changed.
Operational evidence
Approved telemetry and operational records can provide continuing evidence about system behavior, incidents, controls, and conditions relevant to certified scope.
Governance and oversight
Monitoring can surface events that warrant renewed human review, but organizational controls, accountability, documentation, and governance still require evaluative judgment.
Certification status
A significant signal may trigger review, additional evidence requests, re-assessment, or a status action. Telemetry alone does not automatically create or revoke certification.
Evidence ecosystem
Qualified evidence can come from more than one technology.
Clause5afe is not designed around the premise that one proprietary telemetry stack can prove every technical, organizational, documentation, governance, or lifecycle requirement. The certification process can consume qualified evidence from multiple approved sources while Clause5afe controls how that evidence is admitted, interpreted, and used.

From signal to status
Monitoring does not automate institutional judgment.
A significant change or signal can initiate a controlled review path. The response depends on scope, severity, evidence quality, materiality, and the governing certification rules.

A relevant event, change, anomaly, incident, or new evidence is identified.

Qualified personnel determine whether the signal is relevant and material to certified scope.
03Evidence
Additional records, context, or targeted assessment may be required.
04Decision
Clause5afe applies the governing process for continued status, re-assessment, suspension, withdrawal, or other action.
Included capability
Monitoring availability is built into certification pricing.
Every Clause5afe certification includes the client's option to activate continuous monitoring for the certified AI system during the certification validity period at no additional charge. Activation is optional; choosing not to activate monitoring does not reduce the certification fee.

Public boundaries

Monitoring does not guarantee that an AI system is safe, compliant, or error-free.
A monitoring signal is evidence to be evaluated, not an automatic certification finding.
Clause5afe retains authority over evidence admissibility, evaluation, re-assessment, and certification status decisions.
The client remains responsible for operating, governing, changing, and remediating its own AI system.
Clause5afe does not convert monitoring into consulting, implementation guidance, or remediation services.
